Community Health Workers

A community health worker is a frontline public health worker. The community health worker is someone who is a trusted member of your community, which enables them to understand your needs, link you to services you may need. They can be your liaison between health/social services and your community. Community Health Workers assist consumers with access to services, and help improve quality as well as cultural competence among providers and the community. Community health workers can help you by:

  • Increasing your health knowledge
  • Providing education
  • Social Support and advocacy
  • Linkage to needed community resources
