Diabetes Self-Management Program – Workshop

LORHN staff are trained Leaders for the Chronic Disease Self-Management Program (CDSMP), an evidence-based program originally developed at Stanford University. Workshops are offered to rural residents for free, in six-week series with up to 15 participants meeting weekly for 2.5 hours. This workshop focuses specifically on diabetes self-management for the full six weeks. Though much of the content and skills taught are similar to the Chronic Disease Self-Management workshop, there are also variations specific for individuals living with diabetes, for example:

  • monitoring
  • avoiding complications
  • stress management

Spouses, partners, caregivers, and adult children are welcome to join participants in the series.

Beginning in July 2020, all workshops will be offered virtually due to COVID-19. Complete our interest form to let us know your availability and contact information to send you an invitation for an upcoming workshop. If you have a group of 8-12 individuals we will work you with you to coordinate a private workshop series as well.
