Diabetes Self-Management Program – Workshop

LORHN staff are trained Leaders for the Chronic Disease Self-Management Program (CDSMP), an evidence-based program originally developed at Stanford University. Workshops are offered to rural residents for free, in six-week series with up to 15 participants meeting weekly for 2.5 hours. This workshop focuses specifically on diabetes self-management for the full six weeks. Though much of the content and skills taught are similar, there are also variations specific for individuals living with diabetes e.g. monitoring, avoiding complications, stress management. Spouses, partners, caregivers, and adult children are welcome to join participants in the series.
