Critical Access Hospital/Hospital Improvement Innovation Network (HIIN) Program Monitoring

LORHN, under the direction of the Florida Department of Health, is charged with monitoring, evaluating, and providing assistant services to Critical Access Hospitals (CAH) participating in the Hospital Improvement Innovation Network (HIIN) Program within the LORHN Network.

What is a Critical Access Hospital?

Hospitals designated by the Centers for Medicare and Medicaid Services due to the facility’s essential services in the rural community. Hendry Regional Medical Center is the only CAH in the LORHN service region.

What is the Hospital Improvement Innovation Network (HIIN) Program?

A program developed by the Centers for Medicare and Medicaid Services to allow for collaborative learning and improvement across a variety of measures critical to improving the value to the care delivered to patients and is comprised of hospitals, healthcare providers, and the broader caregiver community. The targeted goals of achievement are to:

  1. Prevent hospital-acquired conditions
  2. Prevent complications during a transition from one care setting to another
  3. Decreasing readmissions
